death as a reaper in long black robes has specific details about him that have never been set.
death is between 7 and 13 feet, always about 6 inches taller then the victim. he must always be taller then the victim, because the victim must feel a certain sense of intimidation.
now about death's wardrobe....
death wears the standard basic, simple long, black, ragged robe. now even though the robes are ragged, we have no idea of his body.
either death is a) rotted skeletal remains, the flesh blackened and thin, cleaning to the bones, or b) a mass of energy that either has developed a consciousness or is a lower form or a greater consciousness.
as for his head... well we never see it, no matter what....
i would imagine that he has no face. either a skull or... nothing. blackness. but, as death's hood never drops and he always wears a robe, it doesn't really matter as we are not likely to find out.
